Given one of the issues with my daughter's BPD is inappropriate relationships, is it appropriate for the psychiatrists and therapists to have her call them by their first name?   For example instead of Dr. Wall, Jim.

I don't normally visit this board, but saw you hadn't received a response.  My children call all their counselors, etc. by their first name.  Even their pediatric dentist is know by his first name to all the children.  I think that is normal and helps the child to feel comfortable with the person.

I taught my daughter from a very young age to show respect to her elders, such as the parents of her friends, by preceding their first name with a Mrs, Mr., or Miss.

ie... .Mrs. Holly, Mr. John

She naturally continued this when she dealt with professionals.  Using their professional title.

ie... .Dr. Tom, Nurse Laura

When she entered therapy (a very intimate relationship) she called her therapist Mr. "Bob".
