Your "Small" achievements are really big ones! Remember to take it easy on yourself. For me it seems that your week plan is pretty hefty. I don't know where you are in your healing process, and how used you are to meditation. For me 10 min of meditation is a really long time and what I can manage every day. I suggest you spread out your goals of the week a bit more so you have the energy to actually do it. The good thing about achieving is to succeed. And taking tiny baby steps ensure that you stay on a success streak. And never ever berate yourself if you fail to meet your goals. Just reassess the timeline and pick yourself up, don't stop anything, keep succeeding in small steps! Every time you do something, anything, wallow in pride and compliment yourself, "high five" yourself

It will feel odd in the beginning, but just "fake it", it will feel really good after a while. When you look at your achievements through the lens of your former self who had no problems doing those things in the past, remember that you are in a different situation now, and the greater the challenge of doing the small things the achievement is equally as big as the challenge. Its not so much WHAT you do as much as THAT you do it.
I just wanted to comment through my experience with getting things done. I don't know your situation, but for me your goals of the week would be too much for myself.
I have been helped by learning about "kaizen". Here are some articles:
